Clearance information for All Student-Athletes

The following information pertains to the Athletic Medical Clearance process for incoming first year, transfer, and upperclass walk-on student-athletes.  This process is sometimes confusing, so please read through this information carefully and completely.  It is your responsibility to complete this paperwork correctly and on time.

MEDICAL CLEARANCE

An athlete is considered medically cleared if all completed medical forms have been turned into the athletic trainers and the orthopedic screenings and Impact/BESS tests are completed, signed, and on file in the Athletic Training Room. All Sports Medicine forms are due at least 2 weeks prior to the first practice session.

Returning Student Athlete Check list


You do NOT need another Physical Exam for Clearance

  1. Update Personal Information into ATS (Click here)

    • Access Account (ID:Student ID, Pass: knights)        

    • Update Personal Info (email, cell phone, address)

    • Update Medical History

    • Update Insurance Info

    • Update Emergency Contact Info (2 please)

    • Complete "Athlete Forms"

      • Preparticipation Questionnaire

      • Concussion Statement

Sports Medicine Forms and Medical Clearance 2013-2014

MEDICAL FORMS ARE DUE IN THE ATHLETIC TRAINING ROOM AS SOON AS POSSIBLE.

DO NOT return any forms to the Health Services Department

Below you will find the appropriate medical forms for participation in intercollegiate athletics at the University of Bridgeport. All forms and online information should be filled out completely and legibly. Missing or incomplete forms will delay the participation of the student-athlete in their chosen sport. After returning the forms, athletes will be ineligible for participation for up to 2 weeks. This gives our staff adequate time to review the information and evaluate the medical information. Also, be aware that any student-athlete without completed medical forms will not be covered by our insurance policy and cannot be treated by our staff. All forms need to be completed before any participation in intercollegiate athletics. This will assist in avoiding large summer mailings and ensure that all athletes will have access to forms. Please remember that signatures are necessary, so the student-athletes must send these forms back to us via U.S Mail, FAX, or Email.

  • You should NOT complete the General Student Physical Exam sent by the school it is not the same.
 
  • The Physical Exam is sent to the Athletic Training Room, and then we forward it to Student Health Services

 2. ATS Website information that needs to be completed by the Athlete    

Creating an "NEW"Athlete Profile:

1.  Athlete ID will be:  new

2.  Password will be:  new

3.  Click "login"

4.  Comlete as much of the form as you can, then click "save"

5.  Click on "Athlete Information" icon and complete all the information requested under each tab.

Current Athlete Login:

1.  Athlete ID will be:  your UB student ID #  (without the "0")

2.  Password is:  knights

          GENERAL PERSONAL INFORMATION: Must be completed or updated by all athletes yearly. 

INSURANCE INFORMATION: Must be completed or updated by all athletes yearly.   

          CONTACTS INFORMATION: Must be completed or updated by all athletes yearly.  

ATHLETE FORMS:

               1.  PRE-PARTICIPATION QUESTIONNAIRE: Must be completed by all athletes yearly. 

               2.  STUDENT ATHLETE CONCUSSION STATEMENT: Must be completed by all athletes yearly.

               3.  SICKLE CELL WAIVER / ACKNOWLEDGEMENT: Must be completed by all athletes the first year.

 

3. ImpactOnline Concussion Exam    

This test is required for all first year and transfer students participating in contact sports. The Sports Medicine staff will set this up at the same time as the pre-participation orthopedic screening. This is a baseline exam testing mental status and helps as an evaluation tool in the event a student athlete sustains a concussion during athletic participation at the University of Bridgeport. The test takes approximately 20-30 min.

TURN YOUR POP-UP BLOCKER OFF or the test will not start

If you do not complete the test in a successful manner, you will be required to retake the test once you get to UB for the rest of the Medical clearances and paperwork.

4. On-Campus Exams / Testing

  1. Balance Error Scoring System (BESS Test):  Baseline concussion balance test
  2. Orthopedic Screen: 1st and 3rd year athlete or 1st year Transfer Students 
  3. Returning Athlete Evaluation: Height, Weight, Blood Pressure, Pulse,


If any of the sections/items are not completed prior to arriving on campus it WILL delay your clearance for participation.  Especially the CORRECT physical exam!
